I. PERSONAL INFORMATION/REASON FOR REFERRAL Ms. Hampton reported that she is forty-four years of age. Ms. Hampton reported that she was born in Gary, Indiana. Ms. Hampton reported that she has lived in Georgia for fifteen years. Ms. Hampton reported that she is single. Ms. Hampton reported that she does not have any children. Ms. Hampton has been required to complete a substance abuse evaluation as a result of a possession of an illegal drug related offense. The evaluation is to determine whether Ms. Hampton is suffering from a Substance Related Disorder and to recommend a course of treatment, if appropriate. The interview was conducted directly and entirely in English, Ms. Hampton’s native language. II. EMPLOYMENT/EDUCATION HISTORY Ms. Hampton reported that she attended Everett Tech College in Atlanta, Georgia. Ms. Hampton reported that she graduated in massage therapy in 2006. Ms. Hampton reported that she has not worked over the past five years. Ms. Hampton stated that she was a student at Gwinnett Tech for the veterinary assistant program. Ms. Hampton reported that she was unemployed, but in school for the last five years. She stated that she has not received any other additional trainings or certifications. III.
